A PHP, short for “partial hospitalization program,” is a level of care that delivers hospital-grade mental health and addiction treatment 5 days a week for about 6 hours a day, while still letting you spend the evenings at your home or sober living facility.
At our facility in Southborough, clients arrive in the morning for a quick vitals log by our nursing staff, and a medication management meeting, potentially followed by a morning mindfulness exercise. The rest of the morning and midday are a fluid rotation of skill-building sessions along with group and individual therapy sessions. In the afternoons, you may go over some of the triggers you experienced that day and how you dealt with them, before heading back to real life, where you’ll undoubtedly test those new skills.

In most cases, you’ll start your day at 9 am, where a nurse will record your blood pressure, pulse, sleep quality, and medication side effects in your treatment plan. You’ll also likely meet with a psychiatrist or other prescribing provider, who will make any needed changes to your prescription antidepressants, mood stabilizers, or medication-assisted treatment that may be needed.[1] By 10, you’re in your first skills group of the day, for dialectical behavior therapy, to get more emotional regulation practice by creating scenarios that you’d face in real life.

You’ll have time for lunch on site to maintain a steady blood sugar level for the afternoon sessions. After that, you’ll start working your way through your afternoon sessions. Every week, you’ll collaborate with case workers and family members to ensure that clinical goals remain closely aligned with home expectations.

Comprehensive Intake
On your first day, our multidisciplinary care team will conduct a complete assessment of your biopsychosocial health. This includes screens for dual diagnosis and creating a comprehensive treatment plan that outlines your therapy goals, medication management, and overall wellness needs.
Structured Day Treatment
Our PHP day program is 6 hours and runs Monday through Friday, 9 am to 3 pm. Mornings always start with a vitals check-in and symptom report, followed by mindful grounding. Clients then move into assorted evidence-based modalities groups. CBT groups dissect negative thought loops that feed substance use.[2] DBT sessions focus on practicing real-world emotional regulation.[3]
Medical Oversight
After lunch, patients meet with prescribers privately to discuss dosage adjustments for symptoms or cravings. Nurses are on hand to monitor vital signs twice daily and identify potential safety issues.
Holistic Blocks
Afternoons bring a subdued pace, where clients rotate through various modules for relapse prevention, nutrition coaching, and even yoga. Psychoeducation covers topics such as sleep hygiene and stress management. This whole-person care management approach blends physical wellness and mental health.
Aftercare & Family Integration
One of the most important aspects of an integrated, engaging, and lasting treatment is getting the whole family on board. Weekly family therapy sessions, either in-person or virtually, help keep clinical goals in step with home expectations. Near discharge, you and your case manager will collaborate on step-down measures and strategies to stay ahead of any potential insurance gaps.
